Who is eligible?
1. Resident of IL
2. BMI of 27+ with an additional weight related comorbidity (diabetes, hypertension, hyperlipidemia)or a BMI 30+ (no additional comorbidity required).
3. Have attempted to lose weight through lifestyle changes without success.
4. Are motivated to make lifestyle changes, including diet and exercise.
5. Are not pregnant or planning to become pregnant.
We will be offering complimentary consultations to determine candidacy if there are questions about eligibility.
How it works
1. Book a complimentary consultation with one of our providers online.
2. Fill out intake forms in advanced of your appointment so your provider can review prior to your appointment.
3. During your appointment your provider will assess if you are eligible for treatment and answer all questions you may have. If eligible, your provider will submit your prescription to be shipped to you in 5-7 business days.
4. Regular virtual follow-ups will be scheduled as you initiate the medications.
5. You'll receive a reminder to book your refill check in appointment every 8-10 weeks.

Many patients successfully keep weight off when stopping medication studies have shown but lifestyle changes are necessary.
Our memberships require a 90 day initial commitment. After 90 days you can cancel with 30 days notice. You will be billed one more time and no refills will be submitted.
It's important to follow a healthy, balanced diet as part of your weight loss journey. Focus on incorporating plenty of fruits, vegetables, lean proteins, and whole grains into your meals, and limit your intake of processed foods, sugary beverages, and high-fat foods.
